East Tennessee Alliance for Animals Foster Application

ETAA provides and pays for everything you and your foster(s) could possibly need. This includes: food, beds, kennels, training pads, trips to the vet, etc.

List ALL MEMBERS OF YOUR HOUSEHOLD: 

Humans: (names & ages, including yourself)

Dogs: (names, ages, breed, & are they altered?)

Cats: (names, ages, breed, & are they altered?)

Do you own or rent your home?
If you rent, has your landlord provided written permission for you to have a dog?
If there are no other animals currently living in your home, will your foster dog(s) have regular contact/ socialization with others? If yes, please describe.
Does anyone in your household have allergies or asthma? If yes, who/kind?
Have you ever fostered before? If yes, please describe.
Are you willing to commit to the typical 3 - 4 week time frame? This may vary depending on situation of the dog(s).
Are you willing to take your fosters to vet appointments, transport, etc?
Are you considering foster to adopt?
Are you willing to foster a litter of puppies?
Are you willing to foster a litter of puppies AND MOMMA DOG?
Would you consider a special needs or handicapped dog that may require medication for permanent (but controllable) condition?
Are you willing to foster a bonded pair?
Do you have a fenced in yard? If yes, please describe.

I/we attest that the information provided on the application is true and accurate to the best of my/our knowledge. I/we understand that all rights of foster dog(s) belong to ETAA and I/we are not to try to sell or give away foster. I/we have retained a copy of this application for my/our records and understand that completion and submission of this application does not guarantee or entitle me/ us to the fostering of a rescue dog.
